Becoming A Resident

Once you have decided to make Autumn Glen your new home, we
will meet with you, family members or other
caregivers to learn more about what you want and need from an adult
retirement community.
We want Autumn Glen to be your home, not just the place where you
live. Our staff is committed to your
health, well-being and control. You will find the other residents
and Autumn Glen staff are waiting to give
you ...a warm welcome.
Before moving into your new home, the following is needed:

Completed application form.

Personal interview is required of all applicants to explain the
details of admissions and is generally a part of a tour which
allows potential residents to meet the staff and other residents
and an opportunity for the staff to ensure that Autumn Glen can
meet your needs.

Completed and signed admission agreement or waiting list
deposit. A separate application is required for each individual
and an application fee of a $500 security deposit is payable
upon approval of each applicant.

Scheduled functional assessment, scheduled social and activity
assessment and evaluation and personal needs evaluation with a
Wellness Nurse. Admission requires information about your
medical history, family and personal history, and information
about your personal finances. An applicant may be put on a
waiting list if a desired apartment is not currently available.

Complete physician history and physical form within the last 30
days including a current chest x-ray and TB test within the
previous 3 months is required.
