Your health and safety are our main concern. Our qualified and caring staff supports your independence, yet provides personalized support and assistance as you need it. There is peace of mind in knowing someone is available when needed.

As a part of your base monthly rent, we provide our daily "well-check" program, whereby our Autumn Glen staff will physically check on each resident to determine they are doing well.

We work with you, your family and your doctor, to ensure your needs are met in a caring, efficient and professional manner. Before you move into your new Autumn Glen home, we conduct a thorough, functional assessment evaluation designed to determine and ensure your physical and mental well-being. If additional assistance or services are required, a personal service plan is created to meet your specific needs. Recognizing that needs and circumstances change, this service plan is reviewed on a regular basis, to accommodate your needs should they change.

We develop, only when needed, a personal service plan that supports your need to remain independent, yet provides the necessary assistance as required. These individualized service plans can be easily arranged for simple things such as medication reminders, morning and evening care, incontinence care, additional daily room checks and special monitoring, to higher levels of care and assistance by licensed and certified professionals.

These personal service plans provide for any agreed upon schedule of services beyond the basic monthly rent and services. Services are generally provided in 15 minute increments and are scheduled in advance. Should you need additional services other than those specified, we are always ready and available to assist you. No resident will be denied service or assistance
because no plan or schedule is in place. Your safety and well-being will come first.
